Care Management Services

As we age, experience chronic illnesses, or simply face recovery from an illness or surgery, managing numerous appointments and medications can become difficult. Receiving help coordinating your care can be an enormous relief for those needing care and for family members giving care. IOA provides several care management services, each staffed by professionals specializing in gerontology, social work, and nursing. Each program offers expertise and knowledge of services that can save a family countless hours of research and calling. Additionally, it is important to note that advice through IOA is independent; we do not receive commissions for referrals.

Community Living Fund

The Community Living Fund (CLF) assists with the transition of individuals who have spent years living in hospitals or care facilities and who wish to return to living in the community. CLF also provides services to individuals living in the community who are at imminent risk of institutionalization.

Linkages

Linkages is a care management program, providing assessment and coordination of services for disabled adults age 18 or older who live in San Francisco and need assistance with some tasks of daily living. Short-term help might include installing grab bars, shopping assistance, or intervening in an emergency situation. Funded by the Department of Aging and Adult Services, Linkages seeks to provide access to needed services to enable a person to continue living independently.

Multipurpose Senior service program (MSSP)

When everyday tasks such as cooking dinner, getting to the doctor, or managing money become more difficult, MSSP is available at no charge to provide needed support, and enable people to continue living in their own homes. Through the care management services of MSSP, a Care Manager or Nurse visits a client at home to discuss his or her needs and the services that might be helpful. With this information, a Care Plan is developed and appropriate services are arranged. The care manager continues to stay in close touch, working together with the client to adjust the Care Plan as necessary.

Based in San Francisco, Multipurpose Senior Services Program (MSSP) is designed for adults 65 years or older living in San Francisco with Medi-Cal coverage. Funded by the State and Federal government, MSSP is administered through the California Department of Aging.

Older Adults Care management (OACM)

Based in Palo Alto, Older Adults Care Management (OACM) is a licensed home health agency providing care management and consultation, as well as quality home care and personal care assistance. OACM's care management services begin with an assessment of the client's needs and wants. The care manager then develops an individualized plan, evaluates the home for safety concerns, and arranges services such as home care, housekeeping, transportation, and meal preparation. Care managers provide medical care coordination, including accompaniment to medical appointments and medication management. A professional and experienced presence at and following medical appointments, the care managers can serve as eyes and ears for long-distance loved ones, offering peace of mind to seniors and their families.

OACM exceeds rigorous standards as a provider of both medical and non-medical care. All care is supervised by a licensed nurse and supervision by a licensed psychologist or licensed clinical social worker is available as needed. OACM also provides specialized dementia care with aides trained in "person-centered" care by the Alzheimer’s Association. OACM is available to all adults who desire care in their home. Services are paid for through long-term care insurance or private pay. Learn more

Support Services for Elders

Based in Marin County, IOA's Support Services for Elders coordinates in home care support, medical care coordination and financial and legal services. Care managers coordinate health care needs and medical appointments, arrange for household support, and assist with filing medical claims. CMFS' professional fiduciary services help clients plan ahead, including arranging power of attorney and providing referrals for legal assistance. Governed by the California Probate Code, CMFS' professional fiduciaries can act as attorney-in-fact, trustee, or designated agent.

Support Services for Elders is available to all adults who desire care in their home. Hourly fees are charged for services. Payment plans are available, and long-term care insurance is accepted for care management services.

IOA's 29th Annual Dinner à la Heart

Dinner a la Heart - IOA Fundraiser for Older Adult Services IOA's annual culinary event! Reserve a table at a selection of fine Bay Area restaurants for dinner on Tuesday, February 7th. Prepay your dinner costs and enjoy a delicious 3 course prix-fixe dinner. Participating restaurants donate a substantial portion of the dinner price to support older adult services provided by IOA. Online reservations are now open! Enjoy Valentine's Day early and support Bay Area seniors!
